Jean Hansen is an experienced city transplant who takes a job at a Down's syndrome institution and befriends Reuben, one of the boys afflicted with the disease. Reuben latches on to Jean, to the dismay of Dr. Matthew Clark, the head of the institution. Le psychiatre, fort bien intertéprété par Burt Lancaster, règne en maître sur l'institut, veillant à ce que le personnel ne déroge en aucun cas aux méthodes qu'il désire appliquer à ses protégés. Après avoir décidé de recruter une musicienne désabusée (formidable Judy Garland, vibrante de sensibilité), il s'aperçoit qu'elle s'est trop attachée au jeune Reuben et estime que ses manières d'agir ne sont pas conformes à ses propres idées de traitement. La référence aux méthodes vantées par Bruno Bettelheim est clairement énoncée lorsque le psychiatre dit à sa nouvelle recrue "Love is not enough!".
Le film (le troisième de Cassavetes) est particulièrement saisissant par le soin apporté aux scènes avec les enfants, en en faisant quasiment un film documentaire.
