Após a morte do Homem de Preto, Roland de Gilead se vê diante de três portas que o levam do Mundo Médio para três épocas diferentes da Nova York do mundo. Em 1987, ele encontra Eddie Dean, um viciado em heroína que está tentando entrar em Manhattan contrabandeando um quilo de cocaína pura. Em 1964, descobre Odetta Holmes, uma negra paraplégica, ativista do movimento pelos direitos civis, cuja mente abriga também a malévola Detta Walker.
